An investigation has been launched after an inmate died while in custody at a private prison.
Jake O’Brien was found dead on Tuesday while he was being held at HMP Forest Bank, a category-B facility in Salford.
His death has triggered an independent investigation by the Prisons and Probation Ombudsman Adrian Usher.
He said the watchdog would look into the events leading up to the prisoner death and publish a final report once an inquest has been concluded.
