The Football Association are set to launch an investigation into the tragic death of Sheffield United vice-captain Maddy Cusack after her family presented the governing body with new information.

Cusack, who was the club’s longest serving player and their first to reach 100 appearances, died at her home in Horsley, Derbyshire, on September 20. Police said there were no suspicious circumstances surrounding her death.

Sheffield United appointed an independent third party in October to carry out a formal investigation into concerns the family had raised about processes and conduct at the club.

That investigation found no evidence of wrongdoing but the FA confirmed they were reviewing that process earlier this month.

Mail Sport understands that FA officials met with Cusack’s family this week, with the talks understood to have been positive.

An inquest into Cusack’s was opened at Derby Coroner’s Court in October and adjourned.

In September, Sheffield United’s women’s first team manager Jonathan Morgan stepped away from his duties after a formal complaint was made over alleged behaviour towards certain players.

Morgan was cleared to return to work after an investigation found no evidence of wrongdoing. He returned to the touchline for the first time when the Blades played Tottenham in the FA Cup earlier this month.

The FA have been approached for comment.
